We just moved. How can I make sure Medicaid has my new address?

You will need to fill out a Report of Change form (H1019) and take it to your nearest HHSC benefits office. You can drop it off, fax it, or mail it in. Find the nearest office by dialing 2-1-1 or search our office locations.

We just moved. How can I make sure CHIP has my new address?
A:
You can call CHIP/Children’s Medicaid toll-free at 1-877-543-7669 and tell us. You will also need to call your child’s CHIP medical and dental plans to make sure they have your new address on file.
